Conditional Logic in Notes/Forms Templates
In this tutorial, you’ll learn how to create templates with conditional logic-based questions. Conditional logic lets you show or hide questions based on answers to earlier questions, making your forms smart and efficient.
Introduction
Step 1: Go to the Template section.

Step 2: Click to create a new template.

Step 3: Name your template (for example, "Intake Assessment").

Step 4: Add your first question about demographic details for the client.

Step 5: Add a second question.
Select Single Choice as the type.

Step 6: Enter the question: Are you married?

Step 7: Add options Yes and No for the question.

Step 8: Create another question using the Dynamic Table option.
Name it Marriage Information.

Step 9: Edit the table to add columns for Names and Status.
Step 10: Set the Status column to a dropdown if you'd like, then save the table.

Step 11: Add another question as a Short Answer: Additional Marriage Details.

Step 12: Link conditional questions so that "Marriage Information" and "Additional Marriage Details" only show when Yes is selected for "Are you married?" Click Link Conditional Questions, then select the relevant questions.


Step 18: Save your template.

Using Your Conditional Template
When you add a note for a client or lead, you’ll see the form. At first, some questions—like the marriage details—won’t appear.
Step 1: Select Yes for "Are you married?"

Step 2: The Marriage Information table and Additional Marriage Details will now appear. Fill them out as needed.
Step 3: If you choose No, those questions remain hidden.

You can do the same conditioning for the siblings section—just be sure to map the conditional logic if you want to control visibility for those questions.
